Please make sure you update the school on any medical or disability requirements for your child by completing the Individual Healthcare Plan Form if applicable.

If your child needs medication during the school day please complete an Administering of Medicine Form. Please note we can only administer medicines that are prescribed by a doctor. Medicines must be brought to and collected from the office by an adult.

If your child is diagnosed with asthma please complete our Administering of Medicine Form and supply a copy of your child's asthma plan provided by your asthma nurse to the school office info@stcuthbert-mayne.surrey.sch.uk.
